GCMC is seeking qualified applicants for the following positions:

H.O.P.E.S. Family Support Worker / Care Coordinator
-32 hours per week M-F, benefit eligible includes early evening and occasional weekend work
-Duties include: in-home visitation and care coordination for parents and children aged 0-5 years
-Early childhoor and/or family centered work experience preferred
-Data entry and Microsoft Excel skills required
-Excellent communication skills required including general writing

Home Care Aide - Public Health
-Part time (approx 28 hours per week), benefit eligible, weekend rotation
-Must have completed CNA or HCA course.  Please attach copy of certification to application
-Personal Care and Housekeeping duties required
-Must have reliable transportation, good driving record and proof of adequate vehicle insurance
-Experience preferred
