Hamilton and Hoskins Receive Achievement Award

Greene County Medical Center nurse, Allie Hamilton, and Greene County Public Health nurse, Deb Hoskins, received the Iowa Healthcare Collaborative (IHC) Quality and Safety Award for Achievement in Improving Care Transitions at the IHC Patient Safety Conference on March 6.
This award acknowledges all the hard work being done at the medical center in regard to care transitions. Hamilton, case manager in Acute, and Hoskins have been instrumental in improving coordinated care between disciplines.
In an effort to reduce ER visits and re-admissions, Hamilton and Hoskins work together to ensure that patients have all the information they need, including information about available medical center and community resources.
The success of their work is evident month after month. In the past ten months alone, emergency staff have made 55% more referrals to public health. This allows Hoskins and other public health staff to make follow-up visits and calls to check on the well-being of patients.
The Iowa Healthcare Collaborative is a provider-led and patient-focused nonprofit organization based in Des Moines, dedicated to promoting a culture of continuous improvement in healthcare. The organization annually awards providers or organizations that demonstrate outstanding leadership and achievement in patient safety.
